To complete your form, make sure you "Check Out" your basket as though online shopping.
You will know that your form submission was successfully if you receive an email confirmation with a Contribution #.
Please prepare your wiring and/or installation preparation by the Art Drop Off day, Monday, January 9th.
All Submission fields are required if you enter multiple pieces. Fields with unknown information such as title or price will not be accepted.
Submission changes cannot be made after January 9th
You will need to send image files to us after submitting this form for our virtual platforms. Take photographs of your submitted artwork to create image file, and save image file as your last name and the name of artwork (example: "Rhodes-RinconMountain"). Image files must be a jpeg with a file size maximum of 3000 pixels in height or width. An upload link will be sent to you via email upon submission completion.
